Today was the first day of my internship. I told the lady during my interview for this internship that I am interested in attending Graduate school for social work so she thought it was a good idea to have me work with one of the social workers in the agency. My supervisor is a Medicaid Service Coordinator; she is assigned to 26 individuals who have developmental disabilities. Her job requires her to provide assistance and advocacy to these individuals by finding them residency, getting them involved in different programs, providing them with financial support, and getting them the medical support they need.

When I arrived at the office I met my internship supervisor and she gave me information about herself like where she went to school and what her job requirements are. She then showed me around the office and introduced me to some of her coworkers.
8:50am- 2:00pm
To get more familiar with the individuals she workers with, I was asked to file documents for the individuals she had out. Each individual have a chart with all their information, the charts have different sections where the different documents are placed. My supervisor showed me the different section in the charts and where each document was to be placed. While I was filing, I had the opportunity to read through all the charts and see what each individual is diagnosed with. I also had the opportunity to learn about their family situation as well. Most of the individuals that my supervisor works with are diagnosed with mental retardation, PDD, personality disorder, intellectual disabilities, and Down syndrome. I am familiar with most of these disorders because I learned about them in Dr. Putnam’s Neurobiology of mental disorders class and Dr. Sanders Abnormal Psychology class.

I started off the day by filling out LCED forms for each individual. LCED forms are waivers that waivers that are completed by the service coordinator of an annual basis as part of each individuals service plan to waive services for the individuals.

I worked on filing the documents that I did not get to file on the 14th

I attended a 6month ISP (individual service plan) meeting with my supervisor. Her job required her to meet with each of the individuals she is assigned to every 6months to go over what programs they are involved in, what events they have been attending, and to find out if there is anything they would like to change about the services they have been receiving. This meeting was held at the People inc Museum of disability. I met with one of the individuals, his mother, his boss, and his house manager, who are all expected to be at each of this individuals ISP meetings.

We then drove back to the office where I continued to file and read through the individuals charts.

Today I mailed out updated ISP’s to individual’s advocates and the different programs they are involved in. Whenever the individual’s plans change, their service coordinator is required to alert their families and program staffs about these changes or updates.

We visited one of the individuals group home for her ISP clinical. At this clinical I met one of the individual living in the house who was diagnosed with Smith Magenis Syndrome and another who is diagnosed with Angelman’s Syndrome. I am familiar with Smith Magenis Syndrome and Angelman’s Syndrome because I learned about these disorders in my Childhood of Mental Disorders class with Dr Putnam.

I attended a 6 month face to face ISP meeting today at one of the individual’s home. Most of the individuals in the agency live in a group home. Each group home has a house manager and a certain amount of aids depending on how independent the residences are. At this particular individuals meeting, his sister who advocates for him did not show up because there was a snow storm. My supervisor, the individual, the house manager, one of the aides, a behavioral expert, and myself sat in the meeting and I listened as they discussed what the individual have been doing for the past months, his health issues, things he would like to change about his plan, and things he would like to start participating.

My supervisor and I drove to the People Inc Art Experience where one of the individuals attends day program but she did not show up for her ISP 6month meeting. Even though we did not get to conduct the meeting, I was able to get a tour of the building. People Inc Art Experience is a day program where individuals who are accepted are able to do different art works like drawing, coloring, tie and dye, and sculpture. There is also a music program, a drama program, and a dancing program there.

When we got back to the office, I made phone calls to three individuals to have them complete a phone ISP survey. An ISP survey is a questionnaire that each individual in the agency is required to complete every year. This survey allows People Inc to see how satisfied the individuals are about the services that People Inc provides. Parents or guardians complete the survey if the individual is nonverbal.

I completed a referral package for one of the individuals who are interested in enrolling in People Inc Art Experience day program. In order for an individual to get accepted into the program there are a lot of documents that has to be submitted with their application because the program wants to make sure that these individuals are capable to participate in the activities that they offer.

File documents away into the individual’s charts.
2pm-5pm
The individuals that my supervisor works with, lives in either a group home or home with their families. It is required that on every document that is send out in regards to any of the individuals in the agency, their TABS number, Medicaid number, and social security number has to be on the document. Instead of always having to go into the charts to find these information, I was ask to create two contact list; one for the individuals living in the group homes and one for the individuals living at home. One the contact lists I had to include each individuals TABS number, Medicaid number, social security number, phone number, email, and advocate information.

I started out my day by making phone call to individuals to get them to complete a Quality of life survey. Every individual receiving service through the People Inc agency is required to participate in this survey once a year. The Quality of Life Survey is a survey that helps People Inc to see how satisfied each individual is about the services they are receiving. Individuals who are verbal and are capable of making decisions for themselves completes the survey on their own; advocates of individuals who are non verbal completes the survey for them.

I went to the office today to finish sorting through the mails the other service coordinator had ask me to go through the previous day and I also filed the documents she had that needed to be filed in the individuals chart. Filing for her gave me the opportunity to learn about the individuals she works for because I was able to go through their charts and read their individualized service plans.
